Privacy Policy & Notice of Privacy Practices

 

As an ICF Professional Certified Coach, LegitScript-Certified Practitioner, and Recovery Specialist, I am dedicated to maintaining the privacy and security of all client information. This notice describes how personal and health-related information may be used and disclosed, and how you can access your information in accordance with ethical and legal standards.

This notice describes how your Protected Health Information (PHI) may be used and disclosed by Andrea Arlington, and how you can access this information. Please review it carefully.

 

My Legal Duties

I am committed to maintaining the privacy of your health and personal information. This includes any information that relates to your past, present, or future physical or mental health condition and related care services, known as Protected Health Information (PHI).

I am required by law to:
- Maintain the privacy and security of your PHI.
- Provide you with this Notice of Privacy Practices.
- Abide by the terms of this Notice.
- Notify you if a breach occurs that may have compromised the privacy or security of your PHI.
 

How I May Use and Disclose Your PHI

Your PHI may be used or disclosed for treatment, payment, and healthcare operations without your written authorization. Other uses require your explicit permission. The following are examples of how I may use your PHI:

  • For Treatment: I may use or disclose your PHI to coordinate your care or provide services you request. For example, if you are referred to another healthcare provider, your information may be shared to ensure continuity of care.

  • Emergencies: If you experience a medical emergency or require hospitalization, I may disclose your information to medical personnel or emergency responders as needed to protect your safety and well-being.

  • To Prevent Serious Harm: I may disclose your PHI if I believe it is necessary to prevent a serious threat to your health or safety, or that of another person or the public.

  • Legal Requirements: I may disclose your PHI if required by federal, state, or local law. This includes mandatory reporting such as child abuse or neglect, or threats of violence.

Your Rights Regarding Your PHI

You have the following rights with respect to your health information:

  • Right to Inspect and Copy: You have the right to view and request a copy of your PHI, with limited exceptions. Requests must be submitted in writing. Reasonable fees may apply.

  • Right to Amend: If you believe the information I have is incorrect or incomplete, you may request an amendment. I may deny your request if the information is accurate or if I did not create the record.

  • Right to Request Restrictions: You may request limitations on how your PHI is used or shared. I will comply when feasible and in alignment with your safety.

  • Right to Confidential Communications: You may request that I contact you at a specific phone number, email, or mailing address. I will accommodate all reasonable requests.

  • Right to Request Deletion: You may request that certain communications or documents be removed or redacted when permissible.

  • Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with me or with the Secretary of the U.S. Department of Health and Human Services. I will not retaliate.
